Medicare
Depending on your referral you may be covered under one of the following Medicare schemes.
Clients MUST be referred under a specific Care Plan by their GP.
This referral will include the clients Medicare Number, GP provider number and Date of Referral.
There are currently two available Medicare Care Plans:
Enhanced Primary Care (EPC) Plan
Enhance Primary Care Plans dated 2020 onward, will incur of a consultation fee of $75.00 payable upfront, with an immediate Medicare rebate of $53.80.
A client may fall under the Enhanced Primary Care Plan if:
Living with a chronic medical condition (6+ months in duration)
Deemed to present with complex care needs.
Some common chronic conditions covered under EPC include:
Cardiovascular Disease
Hypertension
Hypercholesterolemia
Osteo & Rheumatoid Arthritis
Asthma
Cancer
Diabetes Mellitus
Stroke
Initial and Standard consults take approximately 30 minutes.
The number of sessions on a referral (up to 5 per calendar year) will be determined by the GP and may be dispersed between a range of Allied Health Providers.

Group Diabetes (GD) Care Plan
Group Diabetes Care Plans are bulk billed.
A client may fall under the Group Diabetes (GD) Care Plan if:
They have been diagnosed with Type Two Diabetes (NIDDM)
Clients will be covered for a total of 9 sessions:
One initial consultation
Six group exercise sessions of either Hydrotherapy or Forever Fit
One final consultation
Group exercise sessions will last approximately one hour.
